New Client Information For Mental Health Services
At our Carroll County Location we offer MH services from age 13 through Adults. We treat a variety of concerns including anxiety, depression, PTSD, adjustment disorders, anger/aggression, grief, and more. This location has a therapist that specialize in TF-CBT, Trauma, Military, Greif and Humor Therapy. We provide both individual and family therapy sessions.
There is new client paperwork you will need to fill out which contains general information, office policies and consent forms you will have to sign. There is also a MH questionnaire that you will need to complete as completely and honestly for the client before the initial session. The questionnaire also helps the therapist know what the client is needing. The paperwork has to be completed before the initial appointment. The legal guardian or the adult client must complete the paperwork and attend the initial appointments. The main location must receive all the new client paperwork, questionnaires, along with the adult client's or legal guardian's driver's license and insurance cards before the first visit. Payment arrangements will be discussed during the scheduling phone call.
For MH. After the initial appointment is completed the weekly sessions begins. These sessions are generally scheduled for the same day and time every week so the client has that consistency. The client is generally the only one in the weekly sessions. There is a family session that is usually scheduled once a month. The adult client or legal guardian and therapist decides the exact timing and who will need to attend the family sessions. The sessions are normally fifty minutes long but we ask you allow the full hour just in case the client needs that extra time. This extra time also allows for any questions you may have before or after the sessions. You are more than welcome to ask the therapists questions before and/or after all sessions.
Referrals from the client's PCP is required for the clinic even if your insurance doesn't require one. We work very closely with the families and their Primary Care Physician (PCP).
